All About Me
“All About Me” is a document that must be taken to all hospital attendances. It is very important that ”All About Me” is kept up to date at all times especially when going into hospital. It will enable the hospital medical/clinical professionals to have a greater understanding of the service user needs.
The “All About Me” document will notify the hospital professionals that the service user (their patient) has a learning disability diagnosis and of any other medical diagnosis plus prescribed medication if any.
It will support the hospital professionals to establish the level of support the service user will need whilst in hospital ie eating, personal hygiene, communication, mobility. It will assist the medical team to ascertain if the service user is back to their normal ability prior to arranging/discussing discharge. “All About Me” will also aid with the discharge planning and any follow up appointments.
