Our teams

Assessment

Our assessment team provides comprehensive multi-agency child and family assessments. Our journey of work with children and families begins with undertaking pre-birth assessments. The team focuses on building relationships with families and children in order to understand the child's lived experience and intervene in a way that makes a difference and keeps children safe.

The team works with S17 and S47 assessments and homeless 16 and 17 year olds who require an assessment. The team also conducts assessments for families who have No Recourse to Public Funds.

Family Support

We have three family support teams who work with children who are the subject of child in need, child protection, pre-proceedings or care proceedings. Practitioners build enduring relationships with parents and wider family members, helping them with their own needs and enabling them to better support their children. The teams also work closely with partners to ensure that the families receive tailored support to meet their needs.

Fostering and Adoption

The Fostering and Adoption Teams work closely together to:

  • identify and match children with suitable adoptive families
  • provide post-adoption support to adopted children and their families
  • provide support for adopted adults looking to access their birth records or trace birth family members
  • carry out referrals
  • attend Looked after Children (LAC) meetings
  • carry out statutory child protection processes as required
  • carry out supervision of foster carers

Permanency

The Permanency Team provides high quality, timely management of permanency planning and related activity for social work relating to Looked After Children and manages support, as required, to children in need of permanency including specialist family finding, life story work, creating options to place in the extended family and any other preparation for placement indicated by best practice standards.

Leaving and After Care

The Team works with Looked After Children 16-18 years as well as Care Leavers up to the age of 15 years. This includes working with Unaccompanied and Separated Young People. As well as meeting all the statutory duties to Looked After Children and Care Leavers a central focus of work is preparing young people for their transition to adulthood and supporting and guiding them post 18 years. Care Planning and Pathway Planning enables this work to progress with young people, their families and/or carers.

Children with Disabilities

The Children with Disabilities Team provides high quality, timely support to children and families when a child in the family has a disability. The team supports children, carers and siblings if required. We work with families so that they can access the resources they need to become independent. The team also undertakes safeguarding work should this be necessary. We are a relationship based service and we always strive to achieve the best outcomes for the children that we work with.
